Policies

Appointments can be rescheduled or cancelled with at least 2 business days advance notice. For example, I must be notified that you’d like to reschedule or cancel an appointment scheduled on a Monday by the previous Friday before 1pm. Otherwise, you’ll be charged for the full cost of the visit.

All payments are due in full at the scheduled appointment. I accept cash, check, Visa, or Mastercard.

I accept Blue Cross (Anthem) health insurance. All others may look into the PPO option available with most health insurance coverage. Please contact your insurance and find out what portion of my fees, if any, they will cover. You will be responsible to pay my fees in full at your appointment, but, as a courtesy, my assistant will bill your insurance directly. Any reimbursement due will be mailed directly to you from your insurance company

Confidentiality is my priority. Unless instructed to do so in writing, I will not disclose any aspect of your care or treatment with anyone.* However, I encourage my patients to give their written consent to share information with their other health care providers. This may include other doctors, specialists, psychologists, therapists, etc. This allows us to work as a team and provide the best quality of care.

*Exceptions: in the event I believe my patient is a danger to themselves or others, I’m ethically and legally bound to report certain personal details to the appropriate authorities. If ordered by a judge-issued subpoena, I must release the required records and the contents contained therein.

  • Long follow-up 90-minute consultation $400
  • Second opinion 90-minute evaluation $400
  • Crisis phone intervention $450
